P/n 01403-1300 Rohs/SMPS POWER/PW722S/Rev 1.0 SUNGHO

Blown chip PWM DIP-8, item IC1 and working with her MOSFET. Cover the chips lost.
I need to know the name of the chip. Also desirable scheme of power supply.
